Transportation

Transportation services are offered Monday through Friday for elders living within the service area.

Meal Program

Congregate Meals: offered Monday through Thursday at 5456 Kak Yot Ln, Crandon, WI 54520 (The Caring Place) from 11:00 am to 1:00 pm.

Home Delivered Meals: offered to home-bound elders residing in the service area.

Caregiver Support

Caregiver support services (respite care, training, support groups, etc.) are offered to unpaid family caregivers.

Activities

A variety of activities and outings are offered to FCP Tribal Elders. See upcoming events and past events for more information, or call the activities department at 715-478-4992 (Jason Townsend) or 715-478-4987 (Jenna Paradies)

Other Services

Other services offered by the Forest County Potawatomi Elder Services Department include visiting/telephoning, health promotions, nutrition education, assessments, assistance with letter writing/reading, and more!
